Our office is an “in network” PPO (Preferred Provider Organization) Physician in the Blue Cross Blue Shield network and in the PHCS (Private Health Care Systems) network.  Dr. Kalal is also a (PCP) Primary Care Physician through the Blue Cross Blue Shield HMO Illinois and Blue Advantage program. 

We accept most insurance companies such as CIGNA, Humana, AETNA, and United Healthcare. We also welcome personal injury cases as well as worker’s compensation.

We are a Medicare participating provider and accept Medicare supplemental policies.
